Neck pain is one of the most common musculoskeletal complaints and often follows long hours at a screen, a poor sleeping position, or an injury such as whiplash. This guide explains what is happening and how different approaches compare.

What is happening in the body

The neck, or cervical spine, has seven vertebrae, discs, facet joints, ligaments and many small muscles that support the head, which weighs about ten to twelve pounds. Pain commonly arises from muscle strain, irritated facet joints, disc degeneration or herniation, or nerve root irritation. Forward head posture increases the load on the neck considerably. Most neck pain improves within weeks, but recurrent pain suggests underlying mechanical or postural contributors.

How it connects to the nervous system and brain

The neck houses the spinal cord, the exit points of the nerves that supply the arms, and the upper cervical region where the spine meets the brainstem. The upper neck contains an unusually dense supply of position sensors that report to the brainstem and cerebellum to coordinate posture, balance and eye movement. The upper cervical nerves also connect with the trigeminal system, which is why neck problems can cause headaches, jaw and facial pain.

The medical approach: medication and surgery

Care often includes heat or ice, activity modification, anti-inflammatory medicines, and muscle relaxants for short periods. Nerve pain may be treated with medicines such as gabapentin, and epidural or facet injections are options for persistent pain. MRI is reserved for red flags or radiating symptoms. Surgery, such as a cervical discectomy and fusion or disc replacement, is considered for progressive weakness or severe compression not responding to conservative care.

The physical therapy approach

Physical therapy combines posture correction, deep neck flexor strengthening, scapular stabilization, mobility exercises and hands-on techniques. Therapists also review workstation setup and screen height. Exercise-based care has good evidence for both acute and chronic neck pain.

The acupuncture approach

Acupuncture is recommended in several guidelines as an option for chronic neck pain, and trials show moderate short-term benefit in pain and function. Sessions are usually given over several weeks and combined with exercise.

General dietary approach

An anti-inflammatory pattern with vegetables, fruit, fish, olive oil, nuts and whole grains supports tissue health, along with adequate protein, vitamin D and hydration. Maintaining a healthy weight and limiting smoking support disc health, and regular movement breaks matter as much as food.

The Specific Chiropractic approach

Dr. Hasson focuses on the atlas and axis at the top of the neck. After a detailed history, exam and, when appropriate, upper cervical imaging, a very specific, gentle correction is made ultra-specifically, followed by re-evaluation. The aim is to restore balanced input to the brainstem and reduce mechanical stress through the neck, which many patients experience as less pain and better mobility. Results vary.

When to seek care

Seek urgent care for neck pain after significant trauma, or with fever, severe headache, weakness, loss of coordination, difficulty walking, numbness in both arms or legs, or loss of bladder or bowel control.
