Headaches That Keep Coming Back? Why the Top of Your Neck Matters

Headaches are one of the most common reasons people seek care. Frequent headaches can reduce work, sleep and quality of life, and the cause is not always obvious. For many people the neck plays a role.

What is happening in the body

Primary headaches include tension-type headache and migraine, while cervicogenic headache originates from structures in the neck. Tension-type headaches feel like a band of pressure, cervicogenic headaches typically start at the back of the head and neck and are often one-sided, and migraines involve throbbing pain with nausea and light or sound sensitivity. Medication overuse can itself cause rebound headaches. Poor sleep, stress, dehydration, posture and certain foods are common triggers.

How it connects to the nervous system and brain

The upper cervical nerves C1 to C3 converge in the brainstem with the trigeminal nerve, which carries sensation from the face and head, so irritation in the upper neck can be felt as head pain. The brain's pain-processing networks and the brainstem also play central roles in migraine. Muscle tension from forward head posture and joint dysfunction in the upper neck can add to the pain load.

The medical approach: medication and surgery

Treatment depends on the type. Acute care includes over-the-counter analgesics and, for migraine, triptans, gepants or anti-nausea medicines. Preventive options include certain blood pressure medicines, antidepressants, anticonvulsants, CGRP antibodies and onabotulinumtoxinA for chronic migraine. Nerve blocks help some cervicogenic and occipital headaches. Imaging is used for red flags. Limit pain medicine to avoid rebound headaches.

The physical therapy approach

Therapists address neck mobility, deep neck flexor strength, posture and trigger points, which has good evidence for cervicogenic and tension-type headache. Aerobic exercise and relaxation training also reduce headache frequency.

The acupuncture approach

Guidelines and reviews support acupuncture for chronic tension-type headache and migraine prevention, with benefit comparable to some preventive medicines in trials. It is a reasonable option, often given over several weeks.

General dietary approach

Regular meals, consistent hydration, limiting alcohol and caffeine swings, and keeping a headache diary to identify personal triggers help. Magnesium-rich foods and a Mediterranean-style pattern may help, and supplements such as magnesium, riboflavin and coenzyme Q10 should be discussed with your doctor.

The Specific Chiropractic approach

Dr. Hasson focuses on the atlas and axis, since misalignment in this region can irritate the upper cervical nerves and affect the brainstem pathways involved in head pain. After an exam and imaging, a gentle correction is made ultra-specifically, and progress is tracked. Research on upper cervical care and headaches is promising but limited, and results vary.

When to seek care

Seek emergency care for the worst headache of your life, a sudden thunderclap headache, headache with fever and stiff neck, confusion, weakness, vision changes, seizure or after head injury.
